New Patient Intake Form
In an effort to expedite your check-in process, please complete the New Patient Intake Forms. If you are unable to do so, please plan to arrive 15 minutes prior to your scheduled appointment time to complete these forms in the office. Please remember to bring a valid form of identification, such as a drivers license or state-issued ID as well as your insurance card, if applicable. Spouses/partners are always welcome (and encouraged) to accompany you to your appointment as this provides them with the opportunity to ask our staff any questions they may have.
We request that you wear (or bring to change into) comfortable, form-fitting clothing to your initial appointment. Ex: fitted cotton tank top or t-shirt, leggings, compression pants, athletic shorts, etc. Please avoid wearing dark colors. We use an advanced piece of software to take a series of non-invasive postural photos to further analyze your spinal structure. Loose clothing or light colors make it difficult for the software to accurately analyze your posture. If you are coming in for an exam on a joint(s), please make sure we can access the area. Ex: Wear (or bring) shorts if we are going to be looking at your knee.
